  State statements are restricted to entitled requesters with a direct and tangible interest, including the registrant, qualifying family, guardian or authorized representative. DHSS applies its immediate-family rule to marriage/divorce statements: direct-line relatives and in-laws up to, but excluding, cousins. Other applicants must document their qualifying interest.

  Use the DHSS Marriage/Divorce/Single Status application linked from the ordering page. Mail applications must be signed and notarized; in-person requests need a signed application and photo ID or two acceptable alternate IDs, plus any required entitlement documents.

  DHSS charges $15 per state record and $15 per additional copy. One search fee covers a five-year search; further five-year searches require another fee. Mail the application, required documents, search payment and a legal-size self-addressed stamped envelope to Bureau of Vital Records, 930 Wildwood Drive, Jefferson City, MO 65109. Alternatively, use the state page’s appointment or VitalChek ordering options; VitalChek adds a service fee. Jefferson City mail processing is approximately 4–8 weeks, varying with volume, errors or additional research. County certificate/decree fees are separate.
